Please print what you need from the choices below.

Take your passport with you to each shop you visit; each shop will stamp their square on your passport or give you a sticker for their square. At the end of your hopping, mail the passport by Wednesday, May 15 to the address listed on it to be entered in the prize drawings! Check what type of hopper you are in the top right corner of the passport, legibly write your name and contact information on the bottom, and don't forget to vote for your favorite shop's Shop Challenge Quilt if you visit at least 5 shops.
